Employee Benefits

All newly hired employees or current employees who have made a transition to a full time or part time status are eligible for benefits 30 days after their hire date or status change.

Full time employees are eligible for all benefits and accrue two weeks of vacation per year. Part time employees are eligible for 401(k), vision insurance, vacation, and sick leave. All full time and part time employees shall be eligible for eight (8) holidays*.  Holidays represent eight (8) hours each for full time employees and four (4) hours each for part time employees.

Life Insurance:
Basic Life Insurance is automatically given to all full time employees at no charge. Full time employees may choose to enroll in additional supplemental life insurance, which may be purchased.

  • Services provided by Health Management Associates hospital - 100%
  • One (1) $250 per admission per diagnosis co-pay
  • Inpatient and selected outpatient procedures require pre-certification

Prescription Drugs

  • CVS Caremark provides a separate prescription drug card program for all eligible employees with health insurance coverage.
  • Mail order is available for all maintenance drugs-$50.00 deductible
  • Generic Prescription (must use if available)-$10.00
  • Preferred brand Prescription (if generic is not available)-$30.00
  • Non-Preferred Brand Prescription (if generic is not available)-$45.00

Dental Plan

  • 50 deductible ($150 Family deductible)
  • Preventative Services - 80% / 20% co-pay (UCR)
  • Oral exams once every six months
  • Prophylaxis (cleaning of teeth) once every 6 months
  • Others (refer to Summary Plan Description)

Basic Services after Deductible

  • 80% paid
  • Tooth Extractions
  • Analgesia and local anesthesia
  • Oral Surgery (including removal of impacted teeth) Repair or cement of crowns or inlays

Major Services Covered at 50% co-pay (UCR)

  • Inlays, crowns
  • Initial installation of fixed bridge work
  • Installation of dentures to cover teeth extracted while under this plan
